Personal: The complete legal name of the applicant asking for the temporary licence must be provided. Employment: It is necessary that the full legal name and address of the applicant’s present employer be given clearly. Don’t abbreviate the employer’s legal name. Professional: Provide a list of only the organizations in which membership or registration is held, which have functions like those of the association (i.e., another regulatory body).
